Been having this same issue. It keeps randomly blocking people or unfollowing them too.
It’s actually gotten worse since I posted. Not sure if it’s internet, relays or something else

Maybe a mixture of both?
It could be, my relays are online and haven’t changed recently though, except for paying for Nostr.land
Not sure how to clear the #damus cache so I’ll just see how it goes for now
