Not really; if you request kind:0 it’ll give you what the relays have.
Only caveat would be if you’re explicitly telling it to load exclusive from cache.

I've got a feeling I'm only loaing from the cache then. I'm never explicitly querying for a kind0. Only ever using the getUser() method
but you're doing a fetchUserProfile I assume, right? tha gets a kind:0
Yes it loads out of date profile data even after a refresh.
I cleared the site cache in my browser and it loaded current profile data.
