NIP 87 currently leaves membership up to a single admin. This is a high level of trust. A strong authority can keep culture within a group coherent, limiting the likelihood of a group split. Members can also leak the key with some effort to invite other members. Basically, no guarantees hold. Voting on membership might be an improvement in social terms, but MLS even says below that this would happen at the application layer, not cryptographically, and goes into some complex detail about how to handle member-managed admission.
