I've tried to self-host with bitwarden, was unsuccessful (so far). Either way, I'd need "online" access or it would be pointless to even have a password manager. I need passwords when I'm not at home; I need password sharing with my wife; I store passwords for my (underage) children, etc.

I'm in agreement with your general stance, though, to not trust. It's a measured risk. And, obvsly, I don't store anything related to BTC online. But I lost it all in a boating accident anyway, so it doesn't really matter...
