Haven't tested my multisig cold storage since I set it up.

Should I do a sanity check every 6 months or so to just prove to myself I can still move coins?

False, I DCA into my semi-cold storage with just single sig

Then when it starts to overflow I transfer to multisig deep codl

Semi-cold is when you can still get $5 wrench attacked

Even with a gun to my head even I can't move my multisig coins without traveling to multiple locations

But why wouldn’t just move it to cold storage to be safe?

Yes! Use a cheap offline laptop with storage drives removed, and use Tails OS on a USB to do any testing of cold storage wallets. Everything is wiped when completed.

I read many clients store their keys in remote locations, so they have to get on a plane to get hold of their keys (or even only one key).

To do that every 6 months is quite an effort.
