It should be based on a trusted npubs list, not follows.
It probably still ends up with the same issue, but maybe it makes it less likely? Make more friction to become a trusted npubs. A follow is a single click, and means nothing. Theoretically marking someone as trusted wouldn't be done for someone you just encountered. My trusted list would be reserved for people I know IRL and for people I've had longer interaction with.
Hopefully npubs (that shouldn’t be trusted) don’t find a way to trick and become trusted

uuuffff! i saw a sign of hashtag abuse yesterday. all sorts of vanilla hashtags but advertising sicko content.
Hashtag spam happens often. If you are using Amethyst there is an option to report content, which can help other users avoid those posts.

That would at least require a longer time commitment, making it more difficult. Not perfect, but better.
