I'm not sure it does much as it's a decentralized network and no actual moderators, except in some specific relays.
I've had someone block and report me, thinking I was a ReplyGirl, but it showed up in my notifications. I'm not sure if anyone else actually does anything about it.

I think the shoe on head is more like hazing.
I did appreciate someone who has a picture of a fox as his profile pic add a photo of a fox with a shoe on the head though.
