I guess it depends on what client you're using. Here's a screenshot from snort where you can put in a comment.

Ok that makes more sense. So I’m assuming snort lets you add the comment prior to sending the zap request to the receiving wallet, that way when it gets the invoice back the comment is part of the invoice.
