Showstopper for me. Grapheme can't be installed on every device, especially here because of the unique screen I imagine it can't

You'll be reporting your location, app usage, and god know what else to the google mother ship whenever you use the device sadly.

Android, or AOSP, the Android Open Source Project is an open source operating system that anyone can download and use. People can also customize Android. Examples of this are Amazon, Google, Rabbit, Daylight, Graphene and many more. It's all Android under the hood based on AOSP with varying levels or customization.

So does their custom Android preserve privacy or not?

Depends what you're planning on doing with it. I use an e-ink device that's purely used for when I'm outside in bright conditions. Installed obtanium for a few apps and never logged into anything with any person information. Gets the job done

That's a question for them. We don't know what they've done to their custom OS.
