Tell me more... How can a spammer counter report? Reports are about posts not people, unless you're blocking someone.
Also reports are tied to your account, so people that follow you are more likely to be warned, while people that follow the spammer and not you are less likely (depends on how the client works).
When you report, it shows up on their profile under the "reports" tab, and tells them who reported them and for what reason. This is with the #Amethyst client. It may be different with other clients. So, they read the list of who reported them and send a counter-report in return often for the same reason they were reported for, and sometimes for other reasons, being false reports.
Also reports can be for accounts as well as posts.
They send false reports on your posts randomly right? Or just pick some of your latest posts or target all of your posts...
It will be up to clients to consider this. The web of trust model of trusting your friends more than strangers will likely be useful.
If you have to report someone's posts often, that person will likely not be someone your friends follow.
If you're a public figure/influencer, you might want to use a second account to report people.

I know the report appears on profile but individual posts? I haven't seen them on the post but isn't there a ratio threshold for the hide spammer filter like if you get 10 reports the filters will auto hide you?
As for counter reporting that is a thing also someone can just keep making new npubs & report you on them. I have one user out of spite put 7 reports on my profile under the same npub.
