This would suggest a manufacturer issue, and not have anything to do with individual airlines(of which there are only two in the USA with this plane, Alaska and United.)

The manufacturer chooses to leave door holes because they make the same exact plane in various lengths, and leave extraneous emergency exits where unnecessary instead of making a plane the appropriate length. Boeing doesnât want to make five planes but they want to sell five plane models.
