What client you on? It’s patchy on Yakkihone/Nostrudel/Nostur, even when I’ve muted someone on all three, which I’ve only done with one account and their mug still appears when they’re on their weekly day trip from X.
Sometimes mutes between different clients don't work. The spec has the option for public or private mutes, and some clients support one or the other. I've noticed this will wreck my mute list sometimes, reminiscent of when your follow list gets deleted while using another client.

The unfollow and mute buttons are there for a reason, yet some still act like they’re stuck in a never-ending loop of content they don’t want to see. If they’re unable to curate their own feed, maybe they should reconsider how they’re approaching social media altogether. It’s almost as if the solution is right in front of them, and they’re too focused on complaining to see it.

Some people are new and have only usd the primal client.
I think that it's one of the main reasons people usually don't stay around for long.
They try primal and their entire feed is shirtless crypto scammers trying to trick people into sending them money.
Only reason I'm still here is because I checked out some different clients but a couple months back after only using primal I thought that nostr was just a bunch of crypto scammers and Wall Street bros circle jerking.
