We use it in every part of Italy in a slightly different way, but the meaning is pretty always the sam, and it can be summerise in this:

So.. no definitely we don't use in a positive way like I always see is being used worldwide.. and I laugh a lot everytime some foreign friend makes a joke with me using it, or wants to compliment for something, and I initially have the opposite meaning ahahahah
