Because it seems like he has multiple npubs or at least he's in multiple relays and the Mute doesn't work?

On amethyst I just muted his profile picture and have not seen reply guy since.
Thanks very much! That's good to know. Unfortunately in this case it would have to be for web, macOS or iOS.
I use Primal and Nostr and I always see this guy even if I mute him.
I'm sure there's a solution for this :)
I know many people are connecting to web of trust relays as well. Some are running their own and others are just connecting to what others are running.
How do you mute a profile pick on amethyst?
Add a muted word as the URL or part of the URL of the profile pic.
Amethyst was updated to apply matured words to profile names and profile pic URL as well as the general feed.

But there will be all sorts of intents if nostr ever grows bigger / more significant. Just someone wanting to cause a nuisance for fun, someone who hates nostr, someone practicing making bots etc etc ¯\_(ツ)_/¯
