just not enabled by default because it costs money. Working in making it free for everyone

apps can have such a feature but the protocol doesn't care what the content is.
technically you could post jpegs inside a note encoded in base64 and the app has to support decoding it.
here is an example of a typical apple user who thinks #nostr is an application:
