I like the fact that everyone replied to your GM but nobody replied to your legit and interesting question, to which I was hoping to find answers in the comments.

In general, if given the possibility/opportunity, I wouldn't want the government to know that I just had a child.

Not gonna lie, I was also expecting more actual answers on what would happen if we don’t return this… my wife and I have been harassed by multiple phone calls from since we left the hospital a week ago urging us to fill and return the paperwork. It’s kind of frightening.
