A user can have multiple npubs.

This might eventually be the norm. One ID for professional stuff, one for personal, another for gaming/hobbies, etc.

What comes to my mind here is, how does Nostr handle orphaned npubs? I guess, not at all, right? They simply accumulate over time.

That’s what I would think.. how do they define a user? A npub that consistently posts notes??

I'd rather see it as npub=identity.

The term user has no relevance in these environments anymore. Only as an abstract term like for UX/UI things.

Imagine a shizo having several npubs and each of his personalities doesn't even know of the others 😀
