If you have USD how do you "use the bitcoin directly" without a trade first? Or do you just tell your nocoiner customers to go fuck off?

The companies I work with aren't going to pay me in BTC, and insurance companies aren't going to pay me in BTC. They will however not do business with me to find someone else though. Then I can't exchange into BTC.

I totally agree with you in theory
but the actual pragmatic fact is that people have to inteface with fiat.
the UX is terrible, especially if lots of people were to suddenly start listening to you and do more self-custody (and don't worry, they won't listen) because it gets even worse when the chain is maxed out
