It would have already happened in the 60s or 70s when a computerized control system malfunctioned in an industrial setting, causing the death of a worker and has been happening ever since.
I guess you gotta specify it was a generative deep learning model involved in the "decision"
Sorry for my having so much beef with "AI."
But the generative deep learning models produced by human brains have been hallucinating and killing people for tens of thousands of years. Computers running approximations of the same thing will never be any more accurate than a human brain.
Human brains and AI models have far fewer similarities than differences.
Also there's something (Spirit, Consciousness, Will, Mind, Animus, Daemon, call it what you will...) that humanity has, and in my opinion AI can never have.
You could even argue that clever piping with automatic valves and switches are artificially intelligent. So AI deaths could have been happening ever since the 19th century.
