I know little of the specifics myself (not a historian) so I ask these types of questions to hear new interpretations.

From what I have read, it originated from China and slowly worked its way towards Europe. The Europeans were well aware of its approach since they were receiving news on what regions it passed through. The fear made everyone mad, before, during, and after. Some of the peasantry danced in streets littered with bodies.

The Monarchies had little left to threaten them with.

yes, the great fire of london, 1666, happened because of the filth and trash in the streets, while the plague was raging, as a direct consequence of the Enclosure Acts

wikipedia mangles it and makes it sound like it didn't start until the 16th century but it started at the beginning of the 15th century and by 1666 the bulk of the nomadic farmers of england/wales/etc were driven off their land:
