Whats the benefit of using a dm relay. I assume my dms are on every relay so why would I want to use a relay that only I can download from? I cant visualize the benefit even though Im sure there is one.

Decentralization and Privacy. DMs should just stay on the user's relay as opposed to be sent everywhere.
You can opt to have public relays there, which is essentially the same as having them everywhere, but the default should be to setup your own receivers to minimize what people can track from your messages.

Clients can ignore this, right? They can send whatver they want to whatever place.

No, this is the new DM (NIP-17). Many clients already code it with custom relay settings. The relay list is also a nostr event, so it is stored in the relays them selves.
I knew about relay list, but their purpose? It's kind of easy to doxx yourself just by setting a personal relay. Or inbox.
That's why I challenge the privacy part of this.

Well, that's why we recommend the DM inbox relay to be closed to the user's posts, like inbox.nostr.wine. In their case, anyone can send a message to you, but only you can see and download them.
I get that, I was more concern about the relay IP/hostname.
