I know what you mean about cobblestones. I have a fused right ankle. Walking on rough surfaces like that throws extra strain onto my knees/hips/lower back as well as makes me unstable as I don't know which way my body is going to 'lurch' to compensate for the next step. In my case I found that it was the side to side variation in terrain that caused undue stress rather than the front to back.
