I'd say this is a pretty significant problem. Everyone I've invited to Nostr from my local Bitcoiner Meetup gives up within a couple weeks because they feel isolated.

Pretty much the same handful of accounts dominates the feed and even their low value posts end up sucking up all of the engagement.
