We know 😂. It’s super easy to build stuff on Nostr. That’s also why a lot of apps are broken though. People spin up a billion nostr apps and then a lot of them get neglected.

Typing a colon in a note in Nostrudel brings up the custom emoji picker. Unfortunately Damus doesn’t have support for custom emojis yet. Primal will display them in a note, but not as a reaction.
