I don't understand the need for USDT because if I want USD it's to SPEND IT and no one who dosen't accept BTC accepte USDT.

The only point of USDT seems to trade the volatility of BTC.

I think that's a USA centric perspective and you'll find the use case pretty massively elsewhere
