When I want to talk to my doctor, I don't care which hospital she is working for.

Asking me to figure out where she is at so that I can log into their half-baked patient portal to get my prescription renewed is ridiculous.

This is why we need NIP-82

Also being control who can write and read your health data should be the norm.

Now, it's practically the same as sharing your private keys around wherever you need something related to your health. There are multiple copies and you cannot delete anything.

Estonia has the system where you can see who has visited your data and you can grant / revoke access at will (https://digiexpo.e-estonia.com/healthcare/national-electronic-health-record/).
