dude, these helicopter pilots get distracted so much - I've seen a few videos of them looking elsewhere or doing something else instead of looking straight out the window.
Once there was a helicopter in front of another and the pilot was looking back at the passengers and a head on collision happened.

I was just watching the video the news had on in the live stream. It's SUPER obvious the helo goes dierectly at the plane when they zoom in. The helo lights are at about a 45 - 60 degree angle off the plane trajectory and impacts from the right rear of the plane.
