but when you spend you exchange your Bitcoin for something worthy. When you sell you exchange your Bitcoin for garbage money

it's effectively the same thing.

from a tax perspective, you are selling your btc for dollars then using those dollars to buy the thingy

it depends. Many people sell just to have more dollars in their bank account and not for buying things right in the moment
