Different clients have different implementations, but it's basically a trust score for a given user relative to your follows list.

I once followed a link to a client that scored everyone on the nostr , I can’t actually remember what the scores were based on but I was surprised mine was so high given I never really say that much. I’m sorry I can’t be more help right now , if the link comes round again I’ll tag you in 🫂
