We’ve had a similar morning my friend. Called in to take back a pt from the icu to the OR for bleeding. Had a repair of an aortic dissection overnight, dropped off stable in the ICU with clear pressure goals (MAP 65). This morning, they didnt realize the sedation had been paused and let his systolic pressure get up to 250!! Two fucking fifty!

Immediately started bleeding and we almost had to go back to the OR. Luckily we got his pressure controlled and avoided another procedure. But the state of healthcare in this country is abysmal. Hospitals run by administrators and nurse managers, cant staff properly, tons of turnover, stupid fucking protocols that cause more harm than good, zero focus on preventative health etc, etc.

It’s only going to get worse. New doctors are being trained to think this is “quality.”

And training doctors is now “big business.”
