There is no rule that says nostr must operate on a v4v basis.

The problems creators face is deplatforming and not owning the relationship to their audiences, and that’s what nostr solves or can solve with the right tools.

Yes, financial censorship happens but it’s much more rare than getting deplatformed (don’t quote me on that, an anecdotal opinion).

Creator didn’t mean making everything for free and wait and see who pays for it - that may work on a large scale if you’re famous and people know what to expect, but everyone else usually charges for the things they create. Nostr doesn’t need to “solve” this at all though it’s nice to have bitcoin payments as an option.
