I believe if it is tagged #nsfw it automatically covers the image.

Best thing I know to do is stay off the universe feed and follow accounts based on your preferences. Also using some paid relays help with this as well.
Must pay for relays to have removed nsfw things. And even then it still slips thru occasionally

It aggregates popular free relays and filters out a ton of the spam in them. And it’s very smooth and fast to connect/load.

Also, you can disable specific relays in your global feed (in Damus, at least).
Most of us disable the free ones and then leave paid relays turned on.
Relayable.org is a free/donation-supported relay that also does some high quality filtering
Try both of those together and you’ll notice a significant difference
