I worry that if this continues clients will end up being highly incompatible and this effectively kills the protocol.

Sort of agree although I think that's more or less intended and expected. Unfortunately this often makes it difficult/annoying to switch been clients since they can't agree on things like user relays, mute lists*, followed hashtags, and so on.

* NIP-51 defines mute lists specifically as kind 10000 yet every client I've tried other than Primal uses a kind 30000 parameterized "mute" list.

It is not true of every app. Bitcoin core has a reproducible build process. You can verify the published binary came from the published source. However I don’t think this is possible for iOS apps due to Apple’s signing process.

#asknostr how do I manage followed hashtags on #damus or #snort or other clients? I can only find followed hashtag lists for zapddit and agora and neither of those seems compatible with any other client.
