sending a dm to an npub doesn't make them a member of OPA, we could be sending DM for all kind of reasons, but we get your point

Email is a protocol. Gmail is an application on top of the protocol. Same with Nostr and Clients.
Sending an OTP to Gmail would count as email login.

And If I understand the concept it's not everything on the client side. Relays also have a specific role in the private messaging and users need to have rhem set up.
