Are you using orbot? Try installing and enabling it.

Also before that try force closing the app and see if it works

Installed Orbot and enabled tor - No luck.

Tried force stopping and restarting Amber fresh, but nothing.

Also tried over VPN, and with a SOCKS5 proxy, no notifications.

I haven't actually seen Amber in action beyond local apps, so just to make sure I have the correct expectation with NIP-46 functionality:

I create a new app on Amber, take the ID, paste it on a web client that supports NIP46, receive a notification on Amber to sign event permissions for the client to access - Correct?

Either way, no notifications unfortunately.

When I was testing I just had to install ntfy and it worked.

Will try with a self hosted server and add the option like amethyst

To be clear, I don't think Amethyst has the option to use a self-hosted ntfy server - I think it's setup exactly the same as Amber - Public ntfy.sh server, with a unique topic for the app instance.

Thanks anyway, really appreciate you taking a further look - Happy to perform any further testing as well. If I can get Amber to work with nip-46 it would be huge for me.
