how come in primal, i get two notifications -- a reply and a mention -- but only from certain people who reply to my post?

The short answer is, it depends on how the client that was used for the reply handles tagging the user the reply is being sent to.
If it is just a "p" tag with your public key, then it will only show as a reply. But if it ads "mention" after your public key, you will see it as both a reply and a mention in your Primal notifications.
ah, got it. so it is the responder's client side that still adds it.
