I've unfollowed someone three times in the last few months, and he keeps popping back into my follow list. How does this happen? Presumably a relay rolling back on a backup or something?

You're not able to unfollow people.
Everyone deserves the same amount of followers 🌈
It's for your mental safety.
Since the technical cause can't be identified, I would just blame them and harass them over it until whoever's responsible is embarrassed and losing customers over it (them losing followers, or some malicious nostr dev losing nostr user growth)
