Do you think it's a good idea to integrate this into relays somehow, maybe as a strfry plugin? Would you use one if it existed? Maybe a https://github.com/damus-io/noteguard module?

No Khatru relay that I know of has this kind of problem because none is a free-for-all relay anyone can write to.
Is it not possible to set up a fully public, de-permissioned khatru relay? If so, a module like this would be attractive
Yes, it is possible, but I'm not interested in doing that. It's probably better to use strfry at that point.
In any case khatru doesn't have the concept of plugins, it's just random code you plug anywhere, so should be simple.
For strfry it makes sense to have one binary blob that you just download and shove into your relay config.
I have no experience - yet - with customizing strfry, but I have experimented with khatru's pattern of "random code you plug in anywhere" and found it to be fairly reasonable to deal with.
If you're telling my strfry is better in that regard I should definitely spend some time with it.
