Pick a part of the world where there more or less is no functional government and I'll customise the list for you.

I don't really know of any but possibly some places in Africa, and yes they are a mess. There aren't any long lasting examples in history either. My position is that it wasn't really possible at a scale larger than villages but technology changes that and new things are possible today.
