Is that divot intentional or has it been worn away?
Worn down. Needs to be replaced. I tools the photo so I could send it to the club alpino folks here who manage replacing anchors.

Stainless steel rap rings are nicer. They help prevent this problem
Most of the anchors actually have those too. But for little roadside crags they have these easier stainless snap locks too. But obviously lots of use means they wear out.
