I use NIP-17 DMs daily in multiple clients (Amethyst, 0xChat, and Chachi) and work pretty well. Sounds like a client issue.

Yes, if you use a client that does not support proper DMs you'll have terrible UX. I highly recommend 0xChat for that, it's available in every platform and is the most advanced nostr chat/dm app.
Are you supposed to periodically open it to see if people dmed you?
Amethyst always shows that I have messages but when I go to read them, it's the same old messages I've previously read.
Yeah DMs are a pain on relays. Same for notifications. Without a database it’s hard to keep track of what was seen and what wasn’t. You can use local database but it seems to not always work well.
Yeah I run Citrine on mobile and it doesn't seem to help with that issue. Bit I also don't know what kind of events it even supports as I forgot about it until you mentioned local relays. I hide or turn off a LOT of notifications.
I would hide them entirely in my client if I had the option. At least for now. They've always been a shit show for me.
