The biggest problem in my view will be dealing with undesired content. This includes both spam and illegal material. Since the protocol is permissionless, any relay can join the network. Any free-of-charge relay will be flooded with garbage, forcing people to use paid relays, which will be a turnoff for most people accustomed to "free" social media services
