If youâre on damus, click on the hashtag, itâll bring you to the hashtag page. There should be a âfollow hashtagâ button underneath the title

If you're on primal you would search for the hashtag and then press the "+ add this feed to my homepage" button. Then go to your homepage and click that same button and scroll down to the hashtag.
Depends on which client app you're using.
In android I use Amethyst. And in that as soon as you click the hashtag, on top right corner you'll see Follow button.
