Decentralized means that there can be multiple instances, like Mastodon and Minds, each with their own rules. Bsky.app is Bluesky's main and known instance.

Because there aren't others. Anyone can set up a new instance, there are tutorials on how to.
Yeah and there are Facebook clones on github. People spin them up all the time but they never gain traction.
Probably due to not earning enough trust and people not wanting to switch. See Koo for example, was a new social media during the Twitter debacle, but ended up closed.
