guide for an iphone:
1. click the + icon in damus, to open the post curation tab
2. upload desired image
3. after uploading, hold your finger on the uploaded image, which will offer you to Copy URL
4. Click Copy URL
5. Open desired DM
6. paste the URL to the text box
7. send

I personally just upload in a public post and then long-press the image preview in the compose view, then copy the link in a DM. Its a few more steps but its clear that the photo is not private, which is not possible on nostr yet

For someone to read your DMs, he should have access to your private key.
but anyone can read DM metadata :
Who you are chatting with and when are public information.
The content of the DM is private, and can only be read by the sender and the receiver.
