You subscribe to relays by adding them to your relay list. Relays pass posts around and should a relay runner "censor" content posts will reroute around it.

You follow people and threads or hashtags. Like I follow #nostr so I saw your post.

what does it mean to follow threads here on #nostr? is there any specifically ways to identify threads?

In my nostr client (amethyst) If you click on any #hashtag you get an option to follow in the upper right side.
