If you’re on a browser client, use an extension like nostr:npub1getal6ykt05fsz5nqu4uld09nfj3y3qxmv8crys4aeut53unfvlqr80nfm or nos2x. There are also ways to protect your keys on mobile, and more stuff coming out soon to make things easier.

What does that icon convey to you?
Could you do something like a purple ring around the profile picture if you follow them and a green ring around it if you dont? Just as an alternative to icons. Could use different colors of course. Just an idea.
