Can you use same public key account for that but hide the private relay from public view? That has to be done at client level right. Or somehow authenticate keys from relays side

You can definitely set it to that. Usernames are not unique...it's up to followers to make sure to follow only with real person and identify what's "real" some other way. NIP-05 is one way

Lightning address is centralized unless you set it up and run it yourself. It needs a web server basically connected to own lightning node to create invoices.
