how a push notification server can work? we can implement it on immortal.
It's all ephemeral. When new events arrive, the relay triggers the Push system that each app uses.
It must be run by the same dev running the application because it needs the token ID to connect to the push system.

What's the difference between broadcast & outbox?
Broadcast sends to many outboxes and any other relay that the user wants them to.
Outbox is just a relay that receives and stores events. It doesn't send it forward to other relays.
