Is there an easy way to do it?
Someone had made a tool that you could use to purge inactive accounts but I never tried it because I remember people saying it messed up their follows. I just do it manually.
I try to do it every few weeks. But I used to do it with tools like nostraidvm but it didnāt find everyone and would spit out a list. I tried with noogle filter this weekend but my follow list got rugged. From my talk with elsat, it seems like it was because of the issue with the big follow lists. So I started doing it manually. It got rugged again (it jumps back from ~1300 to ~1600). But Semisolās restore saved me this time š®āšØ
Whatās your criteria for deletion? Inactive for more than 3 months? 2 months?
My general rule is no posts or replies for a month. I figure at that point itās clear that nostr isnāt really a priority for them. With very few exceptions, I want to follow users who are consistently active. And very large follow lists seem to cause problems with user experience, so me following an inactive npub is essentially like dragging around dead weight from relay to relay.
But I also make exceptions to my 1 month rule, such as if I know we never engage with each other, or if theyāre a little bitch ass hoe, etc.

Most days I have a limited amount of time to spend on nostr. No time for little bitch ass hoes in my timeline or notifications š
To be fair, I chose 1 month somewhat arbitrarily but itās been my consistent guideline. I figure itās normal for people to take time to unplug, go on vacation, get busy with projects, etc. A week or two offline, to me that seems normal. But if it starts pushing towards a month or more, then to me it seems like youāve got better things to do, and thatās perfectly fine and your prerogative, but I donāt want that npub being dead weight, thatās all.

Is there an app to review follow list activity?
I use Nostur and if I tap on a profile pic it gives a little popup that includes ālast seenā which essentially calculates the last time you signed an event to a relay iām connected to. Iām not sure if thereās one that shows it in a more consolidated view for all your follows though. That would be helpful.
